We do baseline HCV Ab VL , ALT/SGPT and repeat HCV VL, ALT, SPGT  at 3 – 4 weeks, if source is not also HIV +.  If source is also HIV + we take it out to 6 months. 

 Our lab is equipped to detect minor levels of virus for both HCV and HIV as our patient population is considered a high risk population.

We do baseline HCV Ab and chemistry, as well as testing for HIV Ag/Ab, and 3 week follow-up with HCV PCR and 6 week, 3 months and 6 months for HIV. Additional chem, cbc, u/a testing if they take PEP and HB testing and Tdap PRN. Please see the attached CDC 2016 algorithm for recommended HCV testing.

I’d like to hear how others are managing post-exposure with a source patient who shows REACTIVE HCV antibody and HCV RNA DETECT/QUANT 200,000+.  Do you typically perform a baseline panel on employee with a repeat in 6 weeks, 4 months, and 6 months.  Other than counseling the patient to practice safe sex and report any symptoms, is there anything else you would recommend?
