We require actual documentation and don’t accept attestations. We have staffing agencies send us documentation for review and we clear or advise what’s needed. We continue to do this as we catch many things
the agencies don’t even when they have staff who are knowledgeable. Couple cases lately of agency sent traveler out for a live virus vaccine when the TB screen hadn’t been completed or sending for second MMR or Varivax too soon so 2nd shot invalid.

List,
Does your organization require actual immunization records and/or titers, drug screens, TSTs/IGRAs, etc., from contract clinical staff? Or, will you accept an attestation from the staffing agency that the individual being placed meets
the hospital’s requirements and that actual records are available for audit?
Thank you,
