The FL Surgeon General issued an advisory about a week ago for “shelter in place” for persons over 65 or with underlying health conditions such as heart, lung, immunosuppression, obesity over 40 BMI, diabetes, asthma, etc.   We  created a Shelter in Place Personal Leave for qualified employees.   . This is an optional leave, so team member may work if they so choose or take ATO time if they choose to go on leave.     Since so many PCPs offices have limited hours, we were provided permission to review EPIC records, with verbal permission by employee, to check medical records for diagnosis.  We do most of these over the phone and are able to ascertain if they qualify for a leave.

We use the same info if they request reassignment, review EPIC records, Agility records or MyChart.  We advise their manager to reassign them if they are pregnant, over 65 or have underlying health conditions from direct care of rule out or confirmed COVID-19 patients.  Our employees may have difficulty getting an appointment quickly so we expedited the process by having our nurse practitioners review their request and medical history, as needed.

Good morning everyone,

Has anyone created a specific medical accommodation (restriction) form for primary care physicians to complete for employees requesting reassignment during COVID?

Thank you,
