At our facility, the COVID-19 vaccine requirement policy went out to all staff on August 16th with an acknowledgement due date of August 30. 
We have 4400 total employees, many of whom are women of childbearing age.
I do not know how many of our employees are currently pregnant.

As of August 31st, we are 75% compliant.

Somewhat surprising, to date there have only been two (2) requests for a pregnancy exemption although, perhaps not as surprising, there are about 250 requests for a religious exemption.

As the Occ Health medical director, I am asked to review requests for COVID-19 vaccine exemptions for validity, however I am not in contact with the employees and I do not access their medical record to protect their privacy.
 
Given the severe risks of COVID-19 during pregnancy, I am requesting that our HR use an informed consent to standardize the information given to our pregnant employees (as well as other community members) by use of an informed consent signed by requester.
 
Does anyone have such an informed consent they are willing to share?
