Very good thoughts.

But, I wonder if the FFR (N95) data are relevant to PAPR/CAPR which has ongoing fan driven flow from the wearer’s face. The CAPR may have a benefit of blocking block direct movement of large aerosol particles (aka droplets).

Also some NPPTL results show that the exhalation valve may not even open at low exertion levels. See:

Roberge RJ, Coca A, Williams WJ, Powell JB, Palmiero AJ. Physiological impact of the N95 filtering facepiece respirator on healthcare workers. Respir Care. 2010 May;55(5):569-77. PMID: 20420727.  “…the exhalation valve is activated only at a certain breathing-pressure threshold that may not have been attained under our study conditions.

An underlying issue is how cautious to be when the data do not clearly say either Yes or No. (ie, what is the right balance for the 'precautionary principle'?)

What still is unknown is whether the unfiltered exhaust air from the CAPR is potentially more hazardous than the exhaled air from the wearer of a surgical mask.  I believe that study of N95s with exhaust valves showed that they offer much better source control than surgical masks, but far inferior than N95s without valves.  The missing variables includes the probability of the HCW using respiratory protection having an unrecognized COVID-19 infection.  I’m not sure that that risk supports not using any particular type of respirator.

Maxair CAPR – incoming air is filtered – exhaust air out (by worker) is not filtered

Most CAPR’s are filtered.  What brand are you using?

Is anyone having staff wear a face mask under a CAPR?  The issue is air blown out is not filtered so the patient may not be protected?
