I attached the grid that we use for Medical Clearance for Respirator use. I also attached our guideline for additional testing. We consider most healthcare workers to be in the low workload/low immediate exposure
risk category.

If there are “yes” answers, employees are contacted by Occupational Health RN or Clinical Assistant with follow-up Questions.
Questionnaire is then reviewed by Occupational Health physician or nurse practitioner. We have a grid for qualification, and recommendation for additional testing based on workload, existing medical conditions
and likelihood for change in status to facilitate consistency.

This is what our Epi Response Team put out here in Tuba City Regional Health Care Corporation, Tuba City, AZ on the Western Navajo Nation. We have never lifted our mask
mandate. This is the most recent guidance.
PPE: Single surgical masks are NOT acceptable for staff in any role.
•
COVID/PUI patients, all dental, swabber in tent, AGP on COVID+ or unknown COVID status: FULL AIRBORNE PRECAUTIONS
•
All other patient contact, including ED: Medical grade KN95/KF94 or fit-tested N95
•
Staff with no patient contact: Medical grade KN95/KF94. Double mask with cloth and surgical ok if preferred.
•
Patients/visitors: KN95/KF94/N95 preferred, or surgical mask. Includes inpatients.

We are using higher level of respiratory protection during aerosol generating procedures, and all our staff at the hospital that cares for patients are wearing
disposable surgical masks, and hospital staff that are not with paitents are wearing either disposable surgical masks or cloth masks. Staff in an office with other people within 6 feet are still masking but if you are in an office alone you can close your
door and take your mask off, and there is a note on the door that says to knock because the person inside may not have their mask on .

We are requiring medical type masks for patients and visitors, not respirators, for the same reasons you outline. Just enforcing the use of medical masks and
not cloth masks or other face coverings is a challenge at times! Interesting that you mention this, in the recent OSHA hearings for informal comment on the proposed COVID-19 healthcare standard, there were proponents of KN95/KF94 masks for universal source
control, to provide better containment without interfering with supply chain for NIOSH-certified N95s needed for healthcare workers.

As the recent variant has come through the community we have seen a significant uptick in cases in the community, the hospital setting and amongst our HCW workforce.
We implemented the use of N95 for HCW who are patient facing (clinical staff and administrative staff who will interact with patients). Now there has been a question raised that all patients should also be wearing n95/kn95/KF94 or equivalent while in the
inpatient or outpatient setting. As a small group (OEM physician, Infection prevention, CNO, director of ED, CMO) we have not implemented the masks for patients. Our reasoning thus far has been the increased stressors placed with the use of these respirators
(especially when you are sick) and potential supply chain issues.
Are there any of you who have implemented N95/KN95/KF94 use amongst the patient population? Any feedback as to for or against use would be appreciated.
