We just implemented  three new intuitive forms via JotForms, accessible to our team members, medical staff, residents and other HCWs.

We also provide QR codes for all three forms so the team members/ other HCWs can complete via mobile devices.  The forms are very intuitive so we don’t need to communicate back and forth constantly which really saves our nurses a  lot of time. We do have a dedicated email for COVID  so we can answer questions as needed.

Our HCWs can RTW as early as day 7 (day 1 is first day of symptoms) after completing the symptom screen, if they meet criteria.    The manager will receive a copy of the clearance also.  We have only been using for one week, but so far it has gone very well.

WARNING: This email came from an external source outside of Tampa General Hospital.

We require call 24-48 prior to RTW for sx review. We tried dropping this and found that we lost control of returning ee. Dates are emailed to manages as potential RTW dates, then another is sent at CLEARED to RTW on date. Specific and black and white.

Hi All:

Bit of clarification to this question: do you require clearance from EHS before RTW after 10 days or do you allow employees to return with their own self-assessment? We want to decrease the volume of phone calls by putting out a decision tree and empowering employees to return without actually speaking with an EHS nurse.

Does anyone know if an actual clearance is required by OSHA, public health, etc? Or have we just gotten in our own way?
