Charlie and Ron,
  You are referencing an impairment evaluation not a fitness for duty evaluation. There is some cross-over.
 Most important we all do these during the day, but off shift ED best resource, but need to contract with drug testing company to come on site and do breathalyzer and UDS if substance abuse is suspected after psych and neurologic causes ruled out.  This is our procedure at several Harvard teaching hospitals. 
The hospital labs and ED's usually botch these UDS and alcohol tests and no COC if challenge.

Can you define exactly what you mean by Fitness for Duty? In my experience these are scheduled evaluations addressing concerns about whether someone is able to return to work. Various documents are gathered and an appointment made, thus we never need to do them off hours. Are you referencing something different?

We have historically utilized our ED to assess employees in the rare event a fitness for duty evaluation is needed in the off hours when Employee Health is not open.

There is a protocol that has been in place.

What do other organizations do?

If substance usage is needed , do you utilize your hospital lab? Our sourced lab?

How do you define roles and responsibilities including manager, HR, EH, EAP and others?
