I cannot speak to a specific institutional protocol but in my private practice, I have been successful in decolonizing patients using a one week course of the combination of the  daily application of pHisoderm (hexachlorophene) body wash from the neck down just before showering and the daily swabbing of both nares with Bactrobam (mupirocin) ointment.
