I didn’t provide the complete form earlier. Please see the details below. We do ask whether the reason for not being vaccinated is religious or medical. However, Employee Health has indicated that we can approve
any statement. The policy states that a declaration of vaccination is “HIGHLY RECOMMENDED.” Employees must either get vaccinated or sign a declination form by November 1st to be considered compliant.
As you can see, this policy is new for us this year and was only approved late in the season. The initial version, which was stricter, was approved on October 17, 2024. After a few credentialed providers expressed
concerns about the strictness of the original policy, we revised the wording. The first version stated that employees were NOT allowed to eat in the dining room or break rooms and would need to take lunch breaks in private vehicles.
Next year, we will need to implement incentives. I also plan to launch a campaign to encourage employees to get vaccinated starting in late August and September. We typically begin employee vaccinations at
the end of September using what we call the “Ring Vaccination” process, which usually enables us to vaccinate 90% of staff within two days, right in their own units.

Appreciate you sharing this. From this declination form, it sounds like the flu vaccine is not mandatory at your facility, so an employee can opt not to get it, with consequences
as you note, but without a formal medical or religious exemption.
If that is true, does your organization offer any kind of positive incentive to encourage people to get vaccinated?
