For those of you involved in reporting employee WPV reporting, do you allow

 

  1. Anonymous reporting with no report to any relevant body outside of the organization:  Our process is they contact the admin nursing sup (here 24/7) to complete a report (mimics questions for CAL OSHA) and a few other questions that our Quality Dept wanted.  We enter names if there was an injury for follow up.  If injured we ask them to also fill out an injury report for WC.  If patient or visitor of patient we have a form that stays on the unit with names of all involved for leader follow up and shift hand off.
  2. Require an employee to enter their name?  Yes if injured and on the unit report for shift hand off.
