Our Employee Health does offsite drug testing with Quest.  We print an order slip with our Quest account number for the new employee to take with them to the lab when they come for their first visit.   The employee then goes to the quest of their choice and has it done. They take the lab slip with them but there is no doctor’s signature on the slip.  This is part of our standing orders.  We do not give any chain of custody slips to our new employees.  All they take with them are the Quest lab slips.    The results are then posted on the quest website.  Our department is billed by Quest each month.   Hopefully, this will help.

Good Afternoon,

Our CAH is starting to conduct offsite UDS for pre-employment , does an order from our EH provider need to be provided? When I called LabCorp, they said only chain of custody form need to be filled out. It seems unusual an order for onsite UDS for pre-employment is needed but not for offsite. Can anyone give some guidance for offsite UDS.
