[MCOH-EH] DOT cards

Hi Geoff,

I don't believe I have any responses from the forum but given that the new 
DOT card is based on the new long form, we are requiring a new DOT 
physical for those who were issued temporary cards prior to 4/20/16. 
As a courtesy, we are charging a subsidized fee, if their initial exam 
(older version of form/ card) was within the previous 30 days.

Question to those who perform DOT physicals: 
If a 30 day temporary card was issued to a driver for them to get some 
medical clearance prior to 4/20/16 on the older version of DOT 
certificate/long form, and they bring an acceptable clearance now, are you 
issuing the new card/certificate based on physical done prior to 4/20 on 
the older long form? Or doing an entire new physical on the new form.
