[CASL-L] generic forms to be in compliance with COPPA

When setting up student accounts on Edmodo, I used the generic form that
Edmodo provides and then tweaked it for my classes (form attached).

A teacher came to me today and asked if I had a generic form that he could
use to set up yahoo accounts for students. I don't. Thought there must be
some forms out there. Did a quick google search and didn't quite find what
I needed either. I looked on CASL ning and didn't find anything there.

I'll share my Edmodo form with him, but does anyone else have a more
generic form?
