[CASL-L] strategic plan surveys

We are in the process of developing a five year strategic plan for our
school library. As part of that, we plan to do a use and needs assessment
survey of students and one of the faculty as well. Has anyone done surveys
like that before and if so, would you be willing to share a copy of your
survey with me? Would like to take a look at lower grade surveys as well,
not just high school. Thanks!
