[CASL-L] Request for Reconsideration

Hi all,
We are looking into updating our Request for Reconsideration form. We have the one from ALA for school libraries but I wanted to check to see if anyone has a different one or had anything to add to that one. Also, do you have a specific procedure to follow in the event of a book challenge?
