[MCOH-EH] Fitness for duty policy

Thanks everybody for your valuable input. We are following the same
procedure here, however sometimes our HR gets confused as to the extent of
the patients' rights to a particular decision so I have had to draft a
policy which clearly outlines each entity's role.
