We do not use a checklist- She does not use a checklist. Our Ergo Evaluator is a doctorally prepared OT with decades of ergonomics specialization. She does a thorough professional history, uses EBP (there is very little to support sit/stand desks), and a detailed physical ergo eval. What has happened in most cases is that other work place modifications have resulted in improvements so that the person doesn't need the desk.
Our experience is that often people want theses desks for reasons that are not consistent with ergonomic need. We have had several people whose managers bought the desk without going through the ergonomic eval and the desk, after a month or so, was removed or the employee moved on and the desk is gathering dust-a very expensive dust catcher.
We contracted last year with this ergo evaluator. One ergo eval costs between $300 and 500 per employee. The ergo evaluator also spent time working with our purchasing department on a limited list of appropriate items for work desk accommodations and these were sent to bid. By contracting with her, we have saved a significant amount of money. She is well worth the cost.

They must have an ergonomic evaluation with our Ergonomic Specialist. If the specialist recommends it, we will purchase it. The ergonomic evaluator has very strict criteria.
