[MCOH-EH] health history and physical exams

We only have employees and volunteers fill out the form, contractors are required to have everything up to date depending on their job. I have attached the form that we use.
We do not do physicals however there are a few positions that we send people for physical capacity exams where the lifting requirements are greater and more frequent or when they are required ot wear a mask e.g.; central processing.

2.       Do you ( NP, PA, Physician) perform a mini-physical on new hires, contractors, students, and volunteers?
