We use a number system.  Source blood is drawn and sent to the lab with hold for OccMed labels and the patient label.  We then identify that this is the correct source, remove all patient identifiers and place a new, generic label with a name of source and number on it.  Those results then come directly to us in OccMed and we match it up with our EE who sustained the exposure.  Our EE's are also drawn under the same source number, but under the name of test.

Regarding the BBPE discussion, may I ask whether you use the real name of employee and source when doing your testing?  If not and you de-identify, what method for de-identification do you use (naming scheme?) and how do you meet the two identifier requirement for the phlebotomist with the lab draw?

We current have a naming scheme and are getting push back from the lab regarding their need for two identifiers when performing phlebotomy.
