Good morning -
At Tampa General Hospital, we use a fictitious name for the exposed employee. For example "Apple, 559". We log names in a confidential and password protected excel spreadsheet. One of the RN's in our clinic draws the labs and labels them with the fictitious name, verifying that the correct fictitious name is used for the correct employee on the consent.
When labs result, we then match the fictitious name to the correct employee and write their name on the print out of the labs. They are then scanned into their chart under the exposure incident.

Regarding the BBPE discussion, may I ask whether you use the real name of employee and source when doing your testing? If not and you de-identify, what method for de-identification do you use (naming scheme?) and how do you meet the two identifier requirement for the phlebotomist with the lab draw?
We current have a naming scheme and are getting push back from the lab regarding their need for two identifiers when performing phlebotomy.
