For required lab identifiers we use the DOB and gender along with the code for staff and for source pt.

At one facility we used a series of questions to develop an alpha code for both staff & source pt. and included the actual DOB & gender on the lab slip & stickers
Currently we use lab slips that have pre-assigned alpha-numeric codes.  We build packets that we use for each exposure.  At point of need we add DOB & gender.
Both systems require a paper tracking worksheet for employee health use only to track what code belongs to whom.

When I worked in the lab in NC we used to de-identify samples of employees for employee health.. all the males were "Jackson, Michael" and a unique number identifier, and the women were all "Maclain, Shirley" with their unique number identifier.  NO one ever wondered or tried to worry about who was who.

Regarding the BBPE discussion, may I ask whether you use the real name of employee and source when doing your testing?  If not and you de-identify, what method for de-identification do you use (naming scheme?) and how do you meet the two identifier requirement for the phlebotomist with the lab draw?

We current have a naming scheme and are getting push back from the lab regarding their need for two identifiers when performing phlebotomy.
