[MCOH-EH] Healthcare RTW Process Questions

Each employee must sign acknowledging their responsibilities as an employee & this is included on that document.


  *   Contact OHS (employee health)
     *   If I missed 3 or more scheduled work days
        *   I am required to have a doctor's note with the RTW (return to work) date & it must include with or without any restrictions.
           *   If upon RTW I have any restrictions, immobile devices, etc., I will be evaluated on an individual bases & per my job description. I understand that these restriction & or immobile devices may not be able to be accommodated.
I must schedule an appointment with OHS prior to the return to work date or I will be sent home and it will be counted as an absence.
