[MCOH-EH] drug screening minors

I am wondering what others follow for a consent policy regarding drug screening minors.  Does a parent/legal guardian have to sign consent or can a minor 14-17 years of age sign for themselves?  Also, are they requiring a cosign by the parent/legal guardian on the Chain of Custody?
