[MCOH-EH] HCWs on Opioids

As part of our on-boarding through Employee Health, we ask about current 
medications.  When there are controlled substances or concerns, we have 
the applicant fill out  a consent to contact the prescribing Provider.  
We send a form and a copy of their job description, asking for input if 
they can safely perform their job while on the medications.
