[MCOH-EH] Survey: COVID Vaccine administration/15 minute observation period

Our "High Risk " clinics have paid off. The employee must wait 30 minutes
and we have an additional level of medical professional support available.
To date we have given approximately 9000 shots and we have had 4
anaphylacic reations including one employee who was hospitalized in the
MICU for a short duration.
